In Calc, right-click a Sheet tab (bottom), and you'll notice "Sheet Right-To-Left" menu-item. It toggles between LTR and RTL, but correct UI behavior should update its state, too, so either (1) add a  check next to "Sheet Right-To-Left", while the document is already in RTL directionality, or (2) change the string of menu-item to "Sheet Left-to-Right", and vice versa, each time a user change doc direction.

Also, consider changing the text to "Right-To-Left Sheet", which is more legible.

Thank you for your feedback – unfortunately important information we will 
need to reproduce your problem is missing.
Please add all information requested in following:
a) Write a meaningful Summary describing exactly what the problem is
d) Attach screenshots with comments if you believe that that might explain the 
   problem better than a text comment. Best way is to insert your screenshots
   into a DRAW document and to add comments that explain what you want to show
f) add information 
   f1) what EXACTLY is unexpected. 
   f2) and WHY do you believe it's unexpected (cite Help or Documentation!)
   f5) concerning your AOO version (with Build/revision ID if it's not a 
      public release) and localization (UI language, Locale setting)
   f6) AOO settings that might be related to your problems 
   f7) how you launch AOO and how you opened the sample document
       (Starts already with RTL?)
   f8) Whether your problem persists after you have renamed your user profile 
     (Quit Quickstart before!) before you launch AOO (please see   <https://wiki.openoffice.org/wiki/Documentation/How_Tos/Handling_the_OpenOffice.org_User_Profile>
     <https://wiki.documentfoundation.org/User_Profile#Mac_OS_X>
   f9) Whether that worked in former OOO / AOO Versions 
   f10) whether (and how) a competitor's software (MSO, LibreOffice, ...)
        fulfills your needs

@Rainer, thanks for reviewing this. 
From our attachment, I understand it's probably a bug on Mac OS only; the MAC version of AOO lacks the "V" which is apparent in your attachment, alongside the  Sheet direction menu-item, in RTL sheets. 

I recommend to try to add a "V" in Mac too, or change the menu item text according to the Sheet's text directionality.

I tested this on AOO 4.0.1, and AOO 4.1 Dev AOO410m14(Build:9760).

(a) What's your Mac OS?
(b) Only for the sake of completeness: you also see the missing 
    checkmark in menu 'Format -> Sheet -> Right to left?
(c) What's with menu 'View'? do you see checkmarks there?

Checkmarks appear properly in Format > Sheet > Right-to-Left, and View menus.
Added Mac OS 10.9.

Its seems that only the context menu from the Sheet title, on bottom, is missing the checkmark, and only on MAC.
